Prior to Starting, You Will Need:
Insurance cards if you would like to use your health insurance for laboratory and pathology services.
Access to your email (for security purposes, you will receive an email with a 4-digit security code to enter during scheduling).

Appointment Types: Skin cancer screenings, acne, rashes and hair loss require detailed, focused evaluations. A dedicated visit is required to thoroughly address these complaints. For example, if you have concerns about both acne and would like a skin cancer screening, please book two separate appointments.
Date of Birth: Select the small “triangle” next to the month and year to select the date
Insurance: If you cannot find your insurance in the pre-populate list, click the blue “ENTER AS TEXT” option

How does a direct care model work?
You pay Conde Dermatology directly for your care without third party involvement. Please see “Pricing” for more details on our transparent fees. Accepted forms of payment include health spending (HSA) and flex spending accounts (FSA), credit cards, and cash. Upon request, we may provide you with a “superbill” to submit to your private insurance company for reimbursement (excludes those with Medicare, Tricare, or Medicaid). Not all private insurance companies allow reimbursement for services. It is the responsibility of the patient to check with their insurance providers regarding rules regulating reimbursement.
All patients will sign a “Private Contract” before being seen for any medical services at Conde Dermatology. For those patients with Medicaid, Medicare, or Tricare you cannot be reimbursed for any services rendered at Conde Dermatology. This however, does NOT change any of your insurance benefits in any way and your insurance will remain in full effect for any services provided by doctors who participate with your insurance. This only means that Medicaid, Medicare, or Tricare cannot be charged or give reimbursement in any way for the services rendered at Conde Dermatology.
Because pathology is not performed in-house at Conde Dermatology, patients may elect to use their insurance (with the exception of Medicare HMOs????) to pay for any necessary labs or pathology ordered by Dr. Conde.
